How Does the Clinic Licensing Process Differ from an Office Permit?
Why does the clinic licensing process differ from an office?
When opening an office, permission is usually obtained from building management and the municipality; when opening a clinic or polyclinic, the Provincial Health Directorate's licensing approval is also required under the Ministry of Health's regulation on outpatient diagnosis and treatment facilities, and this approval cannot be obtained without knowing the space's physical conditions before the project.
Clinic licensing process therefore starts earlier than a standard office permit, usually right after the lease agreement, at the same time as the design.
What is the difference between an office permit and a clinic license?
The table below shows the main difference between the two processes.
| Feature | Office permit | Clinic license |
|---|---|---|
| Approving authority | Building management, municipality | Provincial Health Directorate |
| Physical-condition inspection | Not needed | Inspected by a technical review team |
| Room-based standard | None | Minimum dimensions exist for each unit |
| Design approval timing | Can also come after construction | Must be clarified before licensing |
When this process is placed on a timeline, we covered which stages happen in order from license to opening in a separate article.
How does the physical-condition inspection affect design?
Every unit in a health facility (examination room, waiting area, sterilization area) must meet the minimum physical criteria defined in the regulation; only a small deviation margin is allowed for these criteria. This is why room dimensions in clinic design are clarified while preparing the license application, not during the design stage.
This process also affects clinic design cost; we covered what determines dental clinic design cost in a separate article.
Does this process work the same for every health facility?
No; a private practice, a polyclinic and a medical center are subject to different minimum conditions, which is why the type of facility to be opened needs to be clarified before the project. Starting the design before this is clear may require re-planning the rooms later.
This process can also differ by district; we covered a comparison of three clinic projects in Kızılay, Çukurambar and Mahall Ankara in a separate article.
Umut GÜRLER is an interior architect and a graduate of Çankaya University Interior Architecture; he has worked since 2009 (more than 17 years) on villa, apartment, office and clinic interiors in Ankara, and his office is in Cepa Ofis Kule. The example in this article comes from a clinic project he has completed.
Frequently asked questions about the clinic licensing process
Who approves the clinic license?
The Provincial Health Directorate approves it by inspecting the physical conditions through a technical review team.
Are room dimensions chosen freely?
No, each unit has a minimum dimension defined in the regulation, with only a small deviation margin allowed.
When should design approval be clarified?
Before preparing the license application, the facility type and room dimensions need to be clarified.
Are all health facility types subject to the same conditions?
No, a private practice, a polyclinic and a medical center are subject to different minimum conditions.
What is the biggest difference from an office permit?
The Health Directorate's physical-condition inspection is an additional approval layer not present in an office permit.
